3dfx and Daily Radar are teaming up to offer a $50 rebate on the Voodoo 5 5000 PCI. The offer is valid from now until October 30, 2000 on any variant of the Voodoo 5 (PC/Mac, AGP/PCI). It may be purchased either online or at a retail location based in the United States or Canada. The rebate must be postmarked by November 30, 2000.

The rebate PDF states that this is an exclusive deal for Daily Radar subscribers, but no where on the rebate form do you have to prove that.

With the Macintosh Voodoo 5 being sold online for as low as $300, this may be a good opportunity to upgrade your aging video card to a more modern one for a relatively low price.
